Revival IV ("we", "us", "our") provides mobile IV vitamin and hydration therapy across the Sunshine Coast, administered by registered paramedics. This is our APP Privacy Policy for the purposes of the Privacy Act 1988 (Cth) ("the Act"). It explains the personal information we collect and hold, how we collect, use and disclose it, and how you can access it, correct it, or make a complaint. As a provider of health services, we are bound by the Act and the Australian Privacy Principles (APPs) regardless of our annual turnover.
1. The personal information we collect
Depending on how you interact with us, we may collect:
- Contact details, including your name, phone number, email address and service address.
- Booking details, including appointment times, treatment selections and payment information.
- Health information, including relevant medical history, current medications, allergies and vital signs collected by your treating paramedic before and during treatment, needed to assess your suitability for treatment and deliver it safely.
- Any other information you choose to send us, such as through a phone call, email or enquiry.
2. How we collect your information
We collect personal information directly from you when you book a treatment (by phone, email or through our website), and directly from you during the health screening your paramedic conducts at the start of your appointment.
Where it is lawful and practicable, you may deal with us without identifying yourself or by using a pseudonym, for example when making a general enquiry. Because we deliver an in-person medical treatment, we generally cannot accept a booking or administer a drip without confirming your identity and health details, as this is necessary for your safety.
3. Why we collect, hold, use and disclose your information
We collect, hold, use and disclose your information to assess your suitability for treatment and deliver it safely, manage bookings and communicate with you about appointments, process payments, respond to enquiries, and meet our legal and professional obligations as a healthcare provider.
If you don't provide the information we ask for, we may not be able to safely assess or provide your treatment, or process your booking.
4. Sensitive and health information
Because we provide a health service, some of the information we collect, including your health information, is "sensitive information" under the Act. We only collect sensitive information with your consent, which you give us when you complete your pre-treatment health screening, and only where reasonably necessary to safely deliver your treatment.
We use and disclose your health information only for the purpose it was collected, or a directly related secondary purpose you would reasonably expect (such as clinical record-keeping), unless the Act permits otherwise, for example where required by law or necessary to lessen or prevent a serious threat to someone's life, health or safety.
5. How we hold and protect your information
We store personal information using a combination of secure electronic booking and clinical systems and, where applicable, physical records, and we take reasonable steps to protect it from misuse, interference, loss, and unauthorised access, modification or disclosure. This includes restricting access to staff who need it to do their job and using password-protected systems.
We retain health records for at least 7 years from your last treatment (or, if you were a minor at the time, until you turn 25), in line with standard medico-legal record-keeping practice, unless a longer period is required by law. Once information is no longer needed for any purpose it may be used or disclosed for, and we aren't required to keep it, we take reasonable steps to destroy or de-identify it.
6. Disclosure of your information
We do not sell your personal information. We may disclose it to:
- The registered paramedic attending your appointment.
- Payment processors, to process your payment.
- Our booking and scheduling software providers.
- Government bodies or regulators, where required by law.
7. Overseas disclosure
Some of the third-party providers we rely on for booking, payment processing, email and cloud storage may store or process personal information on servers located overseas, including in the United States. Before your information is disclosed to an overseas recipient, we take reasonable steps to ensure it's handled consistently with the APPs, including through the provider's own contractual privacy commitments. Because this can depend on which specific provider is in use at the time, it isn't always practicable for us to list every country in advance.
8. Direct marketing
We may send you appointment reminders and, if you've opted in, occasional offers or updates about our services, by SMS or email. You can opt out at any time by contacting us using the details below or using the unsubscribe link in the message. We do not use your health information for direct marketing.
9. Data breaches
If we experience a data breach involving your personal information that is likely to result in serious harm to you, we will notify you and the Office of the Australian Information Commissioner (OAIC), in line with the Notifiable Data Breaches scheme under the Act.
10. Website and cookies
Our website may use cookies and similar technologies for basic functionality and analytics. We also use Google Fonts to display type on our site; loading these fonts may involve your browser connecting to Google's servers, which can collect your IP address. We do not run advertising trackers on this site.
11. Access and correction
You can ask us for access to the personal information we hold about you, or ask us to correct it, using the contact details below, free of charge. We will respond within a reasonable time, typically within 30 days, and may need to verify your identity first. In limited circumstances permitted under the Act, we may need to refuse a request for access or correction, in which case we'll explain why.
12. Government-related identifiers
We do not use government-related identifiers, such as Medicare numbers, as our own identifiers for you, except where permitted or required by law.
13. Complaints
If you have a concern about how we've handled your personal information, please contact us first using the details below so we can try to resolve it, and we'll aim to respond within 30 days. If you're not satisfied with our response, you can lodge a complaint with the Office of the Australian Information Commissioner (OAIC) at oaic.gov.au.
